Birth Certificate
Original and photocopy of child's birth certificate
Passport Photos
4 recent passport-sized photographs of the child
Parent ID Proof
Valid ID proof of both parents/guardians
Medical Records
Immunization records and health certificate
Address Proof
Utility bill or rental agreement as address proof
Age Criteria
Child must be 2-6 years old as per program
